Request the reference from me at least 2 weeks in advance of the time you need it. Along with the request turn in a resume to me. Here are some examples/templates to use:

Make sure to include anything in your resume that you would like me to include in my letter.
I will give you both a hard copy and electronic copy of the letter to use for your application.
